- Contact Information (name, email, etc.)
- Membership or Subscription Tier and Term (if you have membership tiers and how long they are signing up to be members. This can vary, based on your membership site)
- How They Found You (this is a great way to see how customers are learning about your business)
Depending on your membership site and what you are selling, the additional items you include in your form may vary. Also, remember to keep your form as simple as possible. You never want to have an overly-complicated form, or else you could quickly turn potential customers off.
